General Summary:
The Pharmacist accurately reviews and interprets medication orders; prepares accurate and complete patient profiles; reviews profiles for correct drug, dosage and route of administration; ensures that no adverse drug duplication or drug-drug interaction exists; ensures that patient allergies are reviewed with respect to any medications prescribed. The pharmacist must efficiently perform other professional duties including, but not limited to, safe procurement, preparation, labeling, distribution and control of all drugs. The Pharmacist notifies the appropriate medical/nursing staff whenever drug therapy is questionable. The Pharmacist is active in department activities, transdisciplinary team activities, and LifeCare activities to ensure individualized, patient-centered health care for all patient populations admitted to LifeCare Hospital.

Patient Population:
Delivers direct patient care appropriate to the patients served. The ages of the patients served are adult (18-65 years of age) and geriatric (65 and older).

Essential Functions:
1. Demonstrates proper performance of skills reflected and validated by the Pharmacist competency checklist.
2. Interprets, clarifies and transcribes all medication orders accurately and timely. Monitors medication orders and reviews patient profile to ensure appropriate drug selection and dosage (no drug duplication, interactions or allergies). Prepares and labels all drugs accurately and timely.
3. Provides accurate, adequate and timely drug information to the professional staff and to patients and their families.
4. Supports the formulary development process by consulting with medical staff and promoting rational drug therapy selection
5. Follows established procedures for drug procurement, storage and distribution, including maintenance of controlled substance records as required by appropriate legislative/regulatory authorities

 

Job Requirements


Experience:
Three years experience in hospital pharmacy preferred

Education:
Graduate of an ACPE-accredited School of Pharmacy or equivalent

Licensure/Certification:
Current Pharmacist license in state where currently practicing

POSITION SUMMARY
The nurse consultant utilizes clinical skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate healthcare outcomes for members.More specifically, Case Management (CM) is defined as a collaborative process that assesses, plans, implements coordinates, monitors and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes. The CM process includes: assessing the member's health status and care coordination needs, inpatient review and discharge planning, developing and implementing the CM plan, monitoring and evaluating the plan and involving the Medical Director as indicated and closing the case as appropriate when the member has met discharge criteria.

We are looking for multiple Registered Nurses to join our care team in Highpoint, NC. The ideal candidate possesses a positive-proactive attitude and will function as a Care Advocate to guide members through complex plans of care that involve clinical, financial, functional, behavioral change and psychosocial components. We seek a Nurse who successfully interacts with members, their families, and providers and in integrating and coordinating resources and services.

This role allows highly skilled Registered Nurses to reach individuals and influence their healthcare outcomes. This nurse needs the appropriate medical knowledge as well as strong interpersonal, influencing and decision making skills. Additionally the Nurse in this role needs to be highly organized, effective in time management, comfortable having his/her work measured, and have strong keyboard and telephone skills. This nurse works in a cubicle environment on a phone queue and with a computer.

RESPONSIBILITIES
Responsible for applying data driven methods of identification of members to fashion individualized case management programs and/or for referrals to alternative healthcare programs
Conducts comprehensive clinical assessments
Evaluates needs and develops flexible approaches based on member needs, benefits plan or external programs and services
Encourages communication, both internal and externally to enhance effectiveness of medical management services and member understanding
Applies case management concepts to complex issues and problem solving techniques in order to promote optimum patient outcomes
Advocates for patients to the full extent of existing health care coverage.
